Case Study – 4-year-old Alex
‘Relocation Case Management’
The Story…
Alex had a diagnosis of Cerebral Palsy. In his home country, he had a team of various teachers, medical and therapeutic professionals who intensively assisted his development. Moving to a new country meant replacing each input, with as little disruption as possible.
The outcome…
After a thorough case history and understanding the inputs and projection from current providers, the Singapore based side of the project began. Medical practitioners ( with specialist experience with Cerebral Palsy) were shortlisted. Local pathways for AAC speech assessment devices were conveyed to the parents and initial assessment appointments were set up. Various therapeutic professionals ( speech therapists, occupational therapists, physiotherapists, feeding therapists etc) were profiled for suitable experience with Cerebral Palsy and shortlisted for parent consideration. Personal introductions were made so the parents could further speak with the therapist to help narrow down the right professional for Alex. We spoke to various pre-school, early intervention centres and schools for their specific experience and ability to cater for a child with Alex’s individual needs. All options were presented to the parents for their consideration. Various other specialist needs ( such as Hyperbaric Oxygen therapy) were also researched on the parent’s behalf.
